It's been a decade or so since I installed a skin head and I don't recall if I did so with the rough side of the head on the inside of the rim, or the outside. Is there a preferred way, or is it just a matter of personal choice, or does it really matter?

The "traditional" way is to install the "grain" side facing up. That is the side that had hair on it (outside). It is a sturdy surface that wears well.
The "flesh" side has a softer surface - with a membrane that might be rough if it wasn't scraped well.
BUT - either way is really fine. I sometimes mount the flesh side facing up on stained heads because the color is often more vibrant.

One more thought. Sometimes it is hard to tell which side is which - especially after the hide is soaked.
If that happens - try this:
Lightly scratch the hide with your finger nail. The softer flesh side will turn white when scratched. The grain side will retain its color.
