Are you sure it's off? Premium ads, as I understand it, are on a round table type format where they come up ever so often (I don't know how often). Perhaps you just haven't seen it as it has come up.

I'm sure it's still a "featured" ad, but if you're talking about the carousel on the homepage under "Marketplace" or at the top of the Marketplace homepage, that just shows 40 random featured ads, and the list changes on every pageview. This is done to keep the page loading quickly.
It still shows up at the top of search results, etc, as a featured ad...
